She has high expectations of this pregnancy. She is in an emotionally vulnerable situation, willing to listen to anyone who can offer her hope. You may be drawn to this vulnerability but it is best that you avoid offering her advice as this case is complicated and you are still a student in obstetrics.<BR/><BR/>To address your concerns, I realize that she has at least three issues here:<BR/>1. Polycystic Ovarian Syndrome<BR/>2. Gestational Diabetes<BR/>3. Recurrent 2nd trimester pregnancy loss, probably due to cervical incompetence. <BR/><BR/>Long-term managament of PCOS is an essential part of patient education here. Though patient feels helpless, she must be made to understand that losing weight remains the key (not while she is pregnant though!). Studies have shown improvement in PCOS patients who exercise regularly, despite not being able to lose weight. In short, exercise helps regardless of patient's ability to lose weight.<BR/><BR/>You may visit RCOG website (www.rcog.org.uk) and look for Clinical Green Top Guidelines on PCOS for further info.<BR/><BR/>As you are in Semester 7, it is important for you to realize the association between PCOS and GDM. You will learn more about PCOS in Semester 9.<BR/><BR/>As per cervical circlage, I feel we should 'cross the bridge when we reach it'. It is definitely too late to subject her to circlage at this gestation. I remembered we discuss a bit on this during a session with u 2 weeks ago.<BR/><BR/>I have some doubts:<BR/>Have interviewed quite a no. of patients who actually having the symptoms of PCOS,and they proble they are facing is getting pregnant,one of the patient i meet actually need to go fertility clinic for help. Her menses were regular until she was 20 +,which out of sudden she gained weight and mentrual cycle become irregular. She had her 1st pregnancy after 5 months of medication for frtility,however,the baby was born very preterm(23 weeks) and coudn't make it. She also had GDM.<BR/>She mentioned that she was quite disappointed when she went back to fertility clinic sometime later after that,but she was told to loose weight first,she felt very helpless(as she just lost her child and she just could not control her weight gain) and thus default follow up at the fertility clinic.<BR/><BR/>My question is: what can i do to educate her?<BR/><BR/>She is currently gravida 3 parity 2 but with no child alive,32weeks of gestational week,currently under close monitoring as in-patient due to cervical incompetence. <BR/>She has asked me whether the cervical incpmpetence will be there forever, as in she will need cerclage for her future pregnancy.
