Its oral first, then nasal. But as I said, the usefulness of this procedure is controversial.

This is what we learnt in neonatal resus. Neonates are obligate nasal breathers. They won't start to breathe until the nasal cavity is clear.
So if we suction the nose first, the baby will take its first breath of air along with the meconium-stained liquor that's still in the mouth.
